A recent Socio-Economic Assessment (SEA) conducted by IPA highlights a crucial opportunity for women’s reproductive health and healthcare systems: the potential role of probiotics in the management of bacterial vaginosis (BV).
The emerging data provide doctors, policymakers, and industry stakeholders with valuable insights into how scientific innovation may contribute to improved health outcomes and hundreds of millions of euros in potential savings for European healthcare.

Understanding the burden of bacterial vaginosis

Bacterial vaginosis is the most common vaginal infection among women of reproductive age, with an estimated prevalence of 23% across Europe and Central Asia. The condition is caused by a profound imbalance in the vaginal microbiome, where protective Lactobacillus species are overtaken by anaerobic bacteria.

Beyond causing debilitating symptoms like itching, burning, and abnormal discharge, BV is associated with broader health implications, including preterm birth, pelvic inflammatory disease, and an increased vulnerability to sexually transmitted infections, such as HIV.

While current antibiotic therapies are an effective treatment in the short term, they present important limitations: up to 80% of women experience a recurrence of BV within a year. This happens primarily because antimicrobials destroy pathogens but fail to recolonize the vagina with the fundamental Lactobacillus species, leading to repeated antibiotic use and continuous treatments.

Research on probiotics and vaginal microbiota balance

Probiotics have been investigated for their potential role in supporting vaginal microbiota balance.. A large meta-analysis by Chen et al. (2022) of 20 randomized controlled studies reported favourable findings associated with probiotic use compared to placebo, as well as improved efficacy when used alongside antibiotics. A finding of great clinical and industrial interest is that oral administration of probiotics was found to be more effective than vaginal administration for achieving persistent colonization.

Several studies have reported encouraging findings. One study demonstrated that the use of a Lactobacillus plantarum strain was associated with higher BV resolution rates: up to 83%, compared to just 48% in the untreated cohort.

Other studies on specific strain combinations, such as Lactobacillus rhamnosus and Bifidobacterium bifidum, have shown a significant reduction in infections during pregnancy and in newborns, while daily oral administration successfully established a persistent colonization of healthy lactic acid bacteria.

The economic impact: an opportunity for policymakers

The costs associated with poor BV management represent a massive burden for healthcare systems. Globally, the economic burden of treating BV is estimated at €5.36 billion.

Translating these BV treatment costs to the European Union, estimates range between €2.07 and €4.14 billion annually. According to the assessment conducted by IPA, if probiotics were included from the initial management stage, increasing therapeutic success from 48% to 83% and avoiding repeated treatments for recurrences, the European Union could save between €0.29 and €0.57 billion every year.

Exploring broader opportunities in women’s health

Current calculations do not account for indirect costs linked to lost workplace productivity and intangible welfare costs related to the discomfort and reduced wellbeing of patients, factors that significantly aggravate the true burden of the condition.

The regular use of probiotics as part of a daily diet may support vaginal microbiota balance and could potentially contribute to broader health and economic benefits

For the biotech and pharmaceutical industry, these findings highlight opportunities for further research and innovation in standardized oral probiotic solutions. For doctors and policymakers, the data speak clearly: integrating probiotics into women’s reproductive health strategies represents an unavoidable step today, capable of combining a better quality of care with much more efficient and sustainable public spending.
